New Client Checklist

• Schedule your first visit by calling 503-667-9457 or emailing dogsncats@dogwoodpet.com. Our first exams are very thorough and run between 30-45 minutes.
• Fill out and bring New Client Form to first visit
• Bring all current medications and supplements to your appointment.
• Ask previous vet to fax full records and diagnostics to 503-669-1659
• Limit food or fast prior to visit. We may recommend testing your pet’s blood and your pet may become car sick.
• Please review our payment options. Payment is due at the time of service. Checks are accepted from established clients only.
• Don’t forget your pet’s leash or carrier for safety.

We understand that unexpected events will arise and you may need to cancel your appointment. Please call us as soon as possible to reschedule, and we will happily find a more convenient time. Time management is extremely important so that we may help ALL of our patients.
